Stop using M4 to generate test program rules

Remove the C_TP, CXX_TP and SH_TP M4 macros that generate Makefile
rules to construct test programs.  Instead, do as we did before: be
verbose and spell everything out -- it's longer, but more concise and
less cryptic.  It also prevents some dependencies from propagating to
targets that don't need them.

This is not as bad as before now that the Makefile is split into
separate files.

Addresses ticket #43.
