Test proper handling of errors when creating the results file

The saner validation of this error case in the C and C++ libraries came
in a previous commit (6aa25b8d86bd92a304ec21edd1374f56077ea658) but did
not include any tests.  The tests come in this change along with proper
validation of this condition in the sh library.

Addresses ticket #45.
